Because dental insurance policies can vary widely between providers and plans, we are only able to estimate your coverage in good faith. Coverage cannot be guaranteed due to the complexity of individual insurance contracts and carrier guidelines.
Your estimated patient portion is due at the time of service. As a convenience to our Jacksonville pediatric dental patients and their families, our office will submit insurance claims on your behalf and allow insurance companies up to 45 days to process payment. After 60 days, any remaining balance becomes the responsibility of the parent or guardian and must be paid in full.
